Our aim is simple:
To help people get home safely, settle back into their routine and regain as much independence as possible.
Helping you return home safely, confidently and with the right support in place.
Returning home after a hospital stay can feel daunting. Even when someone is medically ready to leave hospital, they may need a little extra help to regain their confidence, manage at home and get back to their normal routine.
Beeches Homecare provides rapid-start, flexible home care for adults returning home from hospital or those who have recently returned home and find they need some additional support. We can provide an in hospital assessment to speed up discharge.
A Flexible Service
Everyone's discharge from hospital is different.
Some people may only need a few days of additional support. Others may benefit from several weeks while they regain their strength, confidence and independence.
We can therefore provide different levels of support depending on your needs.
3-Day Support
Ideal for someone who needs a short period of additional help immediately after returning home.
Example:
3 × 45-minute visits per day
This may provide support with personal care, meals, medication routines, reassurance and settling back into the home.
Illustrative price: £255
4-Week Support
Designed for people who need more structured support while they settle back into their normal routine.
Example:
2 × 45-minute visits per day
The care plan can be reviewed throughout the period and adapted as needs change.
Illustrative price: £444*
6-Week Support
A structured support plan designed around personal goals and gradual progression.
This may be particularly appropriate where someone needs additional time to regain confidence and independence following a hospital admission.
Example:
2 × 45-minute visits per day
The plan can include goal setting around areas such as safe mobility, confidence and daily living.
Illustrative price: £2,484*
Ongoing Support
For people who require longer-term care following their period of hospital recovery, we can discuss an ongoing care package based on assessed needs and our usual pricing structure.
Your Support Plan
These packages are illustrations rather than fixed packages.
We can accommodate a combination of visit lengths and frequencies depending on the support required.
Following assessment, we will create an individual support plan with you and, where appropriate, your loved ones and other professionals involved in your care.
Your plan may include:
- Your current care needs
- Your personal goals
- The frequency and length of visits
- Personal care requirements
- Medication support
- Meal and nutrition support
- Mobility and independence goals
- Equipment requirements
- Family involvement
- Communication with other professionals
- Review dates and planned progression
The aim is to provide the right level of support for the right amount of time.
Working with other professionals
A successful hospital discharge often involves several people working together.
Where appropriate, Beeches Homecare can liaise with other healthcare and care professionals involved in your discharge and ongoing support.
This may include:
- Occupational Therapists
- District Nurses
- Hospital discharge teams
- GPs
- Social workers
- Other healthcare professionals
We will work within our professional scope and agreed care plan, ensuring that any equipment or relevant arrangements are in place before care begins wherever possible.
